APPOINTMENTS

To schedule an appointment call (404) 365-0160.� If you are unable to keep your appointment or need to reschedule for a different day, please try to call as early as possible. In most cases you can get an appointment same day you call.

TEST RESULTS

Test Results may take from a couple of hours to a couple of days or longer� before they are ready. In majority of cases nurse may give the information over the phone, but in certain cases Doctor may want to talk to the patient. In most cases laboratory test results are ready at the time of appointment.

A patient's record is strictly confidential. For your protection no records will be released without a written and signed authorization of the patient.

PAYMENTS�
Payments are due at the time of service. Insurance plans may vary. Patient is responsible for any amount that is not covered by Insurance Plan.

From 400 S (toward Downtown): take exit #2 (Lenox Square) to the right. Make left on Piedmont Rd. Cross over Peachtree Street. Continue to go straight on Piedmont Rd. After crossing Pharr Rd. (BP gaz station on the corner), go straight about 100 yards & move left into subdivision "Monteith Commons". We are here.

From I-75 S (toward Downtown): take exit West Paces Ferry RD. Go under the bridge and straight on W. Paces Ferry Rd. which becomes East Paces Ferry Rd. Take right on Piedmont Rd. Then as above
